Each container carries a chain-of-custody log with the date, the person sealing the container, and the person receiving it at the destination. No container is opened in transit, and no container leaves the sealed state until it arrives in the locked records room at the new office.

Sealed Crates and Tamper-Evident Containers

Use plastic crates with serialized tamper-evident seals. The seal number is recorded on the chain-of-custody log; if the seal number does not match at the destination, the breach is documented and reported per the practice's HIPAA incident plan. Never substitute a cardboard box for a sealed crate on a records move.
